A Florida Registered Nurse who ensures that patients admitted to the hospital meet the requirements for acute care admission or observation, as appropriate, and are placed in the appropriate level of care. The Clinical Care Coordinator acts as the primary liaison between the department, the Care Coordination Center/Hub, Nursing, and support departments and in partnership with physicians to ensure accurate patient status and appropriate patient placement for effective, efficient patient-focused outcomes in a timely, appropriate, and cost-effective manner by reducing cycle time, reducing rework, eliminating system redundancies and inefficiencies and immediately escalates patient progression barriers. This person exhibits critical thinking skills, assigns DRG, expected and target length of stay, responsible for the collection and analysis of data to identify opportunities for improvement in the admission process, patient throughput, patient progression milestones, delivery of quality care and educates physicians and staff members about utilization criteria , reimbursement changes and regulations and acts as a role models to staff.
Associate or Bachelors Degree with a minimum of 4 years experience in Nursing. Bachelors degree preferred. Registered Nurse in the State of Florida. Certified in Basic Life Support
